SS-31 calculator

SS-31 is elamipretide, approved by the FDA in September 2025 as Forzinity for Barth syndrome at 40 mg subcutaneous once a day. Trials in heart failure and mitochondrial myopathy used similar daily doses.

About the SS-31 math

SS-31, also listed as Elamipretide, is a synthetic four-amino-acid peptide. Suppliers carry it in 10 mg and 50 mg vials, a five-fold spread, so the two sizes call for different solvent volumes if the units are to match. The example draw is 2 mg. A 10 mg vial with 2 mL of bacteriostatic water is 5 mg/mL. A 2 mg draw is 0.4 mL, or 40 units on a U-100 syringe, and the vial holds five draws. A 50 mg vial with 5 mL is 10 mg/mL, so 2 mg is 0.2 mL, or 20 units, and the vial holds 25 draws. To match the 10 mg vial at 5 mg/mL, the 50 mg vial would need 10 mL, more than most vials hold, so the calculator simply shows a different unit count for each size. With 2.5 mL in the 50 mg vial the concentration is 20 mg/mL and 2 mg is 0.1 mL, or 10 units. A 5 mg draw at 10 mg/mL is 0.5 mL, or 50 units, with ten draws in the 50 mg vial.

SS-31 questions

SS-31 dosage?

The FDA label for Forzinity is 40 mg subcutaneous once a day for Barth syndrome. Community users take 5 to 10 mg a day for 4 to 8 weeks, which has no trial behind it. If the point is mitochondrial support in a healthy person, nobody has shown the low dose does anything.

How many units of SS-31 is 5 mg?

With 50 mg in 2 mL (25 mg/mL) 5 mg is 20 units and 10 mg is 40 units. With a 10 mg vial in 1 mL, 5 mg is 50 units.

How many units is 2 mg of SS-31 at 50 mg in 5 mL?

50 mg divided by 5 mL is 10 mg/mL. 2 mg divided by 10 mg/mL is 0.2 mL, which is 20 units on a U-100 syringe. The vial holds 25 such draws.

How much bacteriostatic water for a 10 mg vial of SS-31?

Any volume the vial holds. 1 mL gives 10 mg/mL, 2 mL gives 5 mg/mL and 3 mL gives 3.33 mg/mL (rounded). A 1 mg draw is 10 units, 20 units or 30 units at those three concentrations.

How many 5 mg draws are in a 50 mg vial of SS-31?

50 divided by 5 is 10 draws. With 5 mL of solvent (10 mg/mL) each 5 mg draw is 0.5 mL, or 50 units; with 2.5 mL (20 mg/mL) it is 0.25 mL, or 25 units.
