long-acting amylin analog · studied for weight loss, appetite control, paired with semaglutide (CagriSema)

Cagrilintide dosage

Cagrilintide is a once-weekly amylin analog from Novo Nordisk, not yet approved. The phase 2 trial in the Lancet tested 0.3 to 4.5 mg weekly for 26 weeks and saw 6% to 10.8% weight loss. In the REDEFINE 1 phase 3 trial it was paired with semaglutide at 2.4 mg each and averaged 20.4% weight loss at 68 weeks. People using research vials usually start at 0.25 to 0.3 mg and climb every 4 weeks toward 2.4 mg, often alongside a GLP-1.

Cagrilintide dose to units chart

5 mg vial + 2 mL bacteriostatic water = 2.5 mg/mL (25 mcg per unit on a U-100 insulin syringe). 5 mg + 2 mL = 2.5 mg/mL, so 0.5 mg = 0.2 mL = 20 units; 0.25 mg = 10 units, 1.2 mg = 48 units, 2.4 mg = 96 units. A 10 mg vial with 4 mL gives the same numbers.

Side effects

Before you start. Not approved, so there is no label dose; the numbers are trial arms and self-reports. Pairing it with semaglutide roughly doubled the rate of GI side effects in REDEFINE 1 (about 80% versus 40% on placebo), and a home-made CagriSema stack has no safety data outside those trials. Amylin analogs slow stomach emptying, so go slow with oral medications and alcohol. Not medical advice; start low and talk to a clinician.

Cagrilintide dosage questions

How much cagrilintide should I take?

Trial doses ran 0.3 mg to 4.5 mg once weekly, and the phase 3 combination used 2.4 mg. Self-reported users start at 0.25 to 0.3 mg and double every 4 weeks or so until they reach 2.4 mg. On a 5 mg vial in 2 mL the 0.25 mg start is 10 units.

Can you take cagrilintide with semaglutide?

That pairing is CagriSema, which Novo Nordisk tested at 2.4 mg of each in REDEFINE 1 with 20.4% average weight loss at 68 weeks. In the trial both drugs were titrated together every 4 weeks. Doing it yourself with two vials has no safety data, and GI side effects were about twice as common as placebo.

Cagrilintide vs semaglutide, which is stronger?

Alone, cagrilintide 4.5 mg gave 10.8% weight loss at 26 weeks, about the same as liraglutide 3 mg in the same trial. Semaglutide 2.4 mg alone gives more. The point of cagrilintide is the combination, where it added several percentage points on top of semaglutide.

Is cagrilintide approved?

No. As of this writing it is only available as the investigational half of CagriSema in trials or as research material. There is no FDA label and no official dose.
